Helpful to have ready: Full name and DOB, reason for visit (cleaning, toothache, etc.), insurance member ID (if using insurance), and preferred days/times.

New Patients
Bring a photo ID and insurance card (if using insurance). If you have recent dental X-rays, you can share those too.
Follow-Up Care
It is widely advised that patients should visit their dentists for evaluation and cleaning at least once every six months. By making annual dental check-ups, your dentist is able to examine your oral health and identify any problems.
